Form 5A

EPFO Form 5A, or the Return of Ownership, is a mandatory employer document submitted to the Employees' Provident Fund Organisation (EPFO) to register an establishment and provide details on its ownership, management, and branches. This form serves as an employer's eKYC and must be submitted digitally with a Digital Signature Certificate (DSC), and its extract is now required to be displayed at the establishment's entrance or on its website for transparency.
